Which former ‘All Black’ died of a heart attack at the age of 40 on 18th November 2015?

An emotional rendition of the ‘Haka’ was performed by his teammates at his funeral. The service was held at the Eden Park National Stadium in Auckland and the traditional ‘Maori’ dance was also performed as his casket was carried out.
